A: こんにちは。しんさつ を うけたいのですが。
konnichiwa, shinsatsu wo uketai nodesuga.
(hello, I would like to see a doctor.)

B: こんにちは。どのようなしょうじょうですか？
konnichiwa. donoyouna shoujou desuka?
(hello, What symptoms do you have?)

A: ねつがあります。おなか もいたいです。
netsu ga arimasu. onaka mo itai desu.
(I have a fever. I have a stomachache too.)

B: ねつはなんどですか？
netsu wa nando desuka?
(How high is the fever?)

A: ３８どあります。
sannjuuhachido arimasu.
(I have a temperature of 38 degrees [Celsius].)

B: わかりました。ほけんしょうは ありますか？
wakarimashita. hokenshou wa arimasuka?
(Understand. Do you have an insurance card?)

B: ありがとうございます。ばんごう で およびしますので おまちください。
arigatougozaimasu. bangou de oyobi shimasu node omachikudasai.
(Thank you. I call you with number so please wait for a while.)

Point

・しんさつ を うけたい want to see a doctor

しんさつ (診察）is a noun meaning “seeing a doctor”. うけたい means “want to see”.

・どのようなしょうじょうですか？

しょうじょう（症状）is a noun meaning “symptoms”.

・ねつがあります。おなかもいたいです。

ねつ が あります means “I have a fever”. ねつ means “fever” in Japanese. あります is a verb means “have”.
おなかがいたいです means “I have a stomachache”. おなか means stomach and いたい means “hurt”.

・３８どあります。

In Japan we use Celsius for temperature.

・ほけんしょう は ありますか？

A receptionist in hospital must ask you this phrase. I mean, she ask if you have an insurance card.
